This is an engorged pointer. It looks like its pointing to the breast area. Notice the pigmented tophi. I would recommend that she have a thorough investigation asap.

Did she lose a lot of weight during her detox? Sometimes fat loss increases excess hormonal release - especially estrogen. (breast link)
